Evaluation of Rigid and Non-rigid Registration in Sequential Quantitative SPECT for Targeted Radionuclide Therapy

Abstracthis study aims to evaluate the merit of non-rigid registration (NRR) over rigid registration (RR) in sequential SPECT scans for improving 3D dosimetric accuracy in targeted radionuclide therapy. We modeled a patient injected with In-111 Zevalin using the digital 4D XCAT phantom, and simulated SPECT acquired at 1, 12, 24, 72 and 144 hrs post-injection. Organs were randomly rotated/translated within 5˚/pixels. Realistic 128 noisy projections were generated analytically over 360º with full physics modelling. A patient injected with 240.5 MBq In-111 Octreo underwent SPECT at 4, 8, 24 and 48 hrs post-injection. A total of 144 projections were acquired over 360˚ with 20s/projection. Reconstructed images were obtained by OS-EM with full compensation. Individual organs segmented from the SPECT images were registered to the 24-hr image using (i) Euler transformation as RR and (ii) affine+b-spline as NRR. Voxel-by-voxel integration was performed on registered images over 5/4 time points. Differential and cumulative dose volume histograms (DDVH & CDVH) were generated for liver, spleen, kidneys and lungs. Normalized absolute errors were calculated from DDVH and it was lower for NRR (7.0-32.3%) as compared to RR (17.8-77.4%) for all organs. The CDVHs for NRR were closer to without misregistration as compared to RR for all organs in simulations. The average computational time of RR was twice as compared to NRR. Results of simulation and clinical study were consistent. We conclude that the accuracy improvement for NRR over RR well offsets its longer computational time. NRR on sequential SPECT is promising for reliable 3D dosimetry.
